HORMONE PELLET INSERTION

Women start at $250 per pellet + office fee (varies if insurance or none)

Men start at $500 for 800-1000mg of Testosterone + office fee (varies if insurance or none)

Note: Labs MUST be done before pellet insertion and monitored after for repeat insertion procedure.

INJECTIONS/INTRAVENOUS (IV) THERAPY

CASH LABS 

Please call to discuss reduced lab rates for cash paying if no insurance or if your insurance doesn't cover out of network physician orders for labs.  Rates are reduced at least 50% of what a standard lab would charge.

IgG/IgE Food Sensitivity Test - Starts at $185 for 95 foods (paid directly to the lab)

If you cannot fill the form out online please print out Intake form below and fill out before your first appointment.  Records release below only if indicated.

A credit card MUST to be kept on file for everyone in case of missed appointments and to cover expenses not covered by insurance.

Questions to ask your Insurance

  • Do you have ND (Naturopathic) Benefits?

    • If your insurance says “preventative” visits are covered, that does NOT mean that you have ND benefits. Preventative coverage applies to annual physicals / PAPs / mammograms and colonoscopys. Please specifically ask if your policy covers “an office visit with a Naturopathic doctor”.

  • Do you have a deductible that your office visit(s) will go towards?

    • If so, usually you will need to meet your deductible before your insurance provider will start paying for the visits.

  • Do you have coinsurance?

    • This is what the insurance will cover versus what the patient’s responsibility is. (Sometimes you’ll need to hit your deductible and then insurance will start paying a percentage of your visit costs. So sometimes there is still a percentage of the cost that is the patient’s responsibility, even after you have met your deductible).

 

Important Note: We ask that you please verify the above information through your insurance provider prior to your visit. There is usually a customer service phone number listed on the back of your insurance card. We encourage you to verify your benefits before your appointment so you are aware of any costs that you will be responsible for.

Late cancellation or No Show fee:

There will be a $145 fee billed for new appointments or $75 for follow-up appointments if you cancel your appointment with less than 24 hours notice.
